Veterans Affairs
All VA forms are available in the Veterans Affairs Office unless noted otherwise. All forms should be submitted to the Veterans Affairs Office at Clayton State University, not to VA.
Active Duty
If you are applying for veterans benefits for the first time
and you were on active duty, you will need to complete a
VA Form 1990 and submit your DD214 (Member 4 copy).
Changing Schools
If you have used your benefits previously and are simply
changing schools, you will only need to complete a VA Form
1995 to change your place of training. You will not need to
resubmit a copy of your DD 214 or DD-2384.
Vietnam Era GI Bill
For Veterans from the Vietnam Era GI Bill will need
to complete an additional VA Form 21-686C and submit a copy of
the marriage license/birth certificates for the spouse and each
child named as a dependent.
Reservist
If you are applying for veterans benefits as a reservist,
you will need to complete a VA Form 1990 and submit a copy of
your DD-2384, Notice of Basic Eligibility (NOBE). The DD-2384 is
obtained from your unit.
Montgomery GI Bill
Veterans receiving Montgomery GI Bill (MGIB) benefits
can use WAVE to submit monthly verification. For

As a Spouse/Dependent
If you are applying for veterans benefits for the first time
as a spouse or dependent, you will need to complete a VA
Form 22-5490. It is sometimes helpful to provide a copy of the
birth certificate for each dependent with the application;
however, this is not required.
