Financial Aid Forms

 

Please do not complete any of the forms or worksheets on this webpage unless you have been instructed to do so by the Financial Aid Office since not all students are required to submit these forms.

In order to ensure that we are in compliance with federal and state regulations, only documents that have been requested by the Office of Financial Aid will be accepted.  You can identify documents that have been requested by viewing the Financial Aid Missing Information Letter on the DUCK.  Questions regarding documents can be forwarded to FinancialAid@clayton.edu.

Please keep a copy of any documents that you submit to the Financial Aid Office as copies cannot be provided for you at a later date.  All documents submitted to the Financial Aid Office must be signed.  Unsigned documents will delay the processing of your financial aid. 



 

Student Rights & Responsibilities


Copies of Tax Returns:  You have two easy and convenient options for getting copies of your federal tax return information (tax return transcript) by phone or by mail.  Request transcripts by calling 1-800-829-1040, or order by mail using IRS Form 4506T (Request for Transcript of Tax Return).   The IRS does not charge a fee for transcripts.   Allow two weeks for delivery.

2010-2011 Forms
Fall 2010, Spring 2011 and Summer 2011 (Please note that all forms must be signed before submission to the Financial Aid Office.)

These PDF versions are fillable.  You can complete the form online and print for your signature.  The forms can also be printed and completed by hand if you prefer that method. Instructions for the fillable forms are below.

Please read the following information as this will help you in completing the fillable application. 

Note that you MUST print and sign each document before submitting to the Financial Aid Office.  Unsigned documents will delay the processing of your financial aid.

  • When you open up the form in Adobe Acrobat Reader or Foxit Reader, the area for each question should be "highlighted" with a color.  If they are not, click on “Highlight Fields” in the top right corner (right above the top of the document) .  Although you can still tab through the items without them being highlighted, it is easier to see where you are.  When you finish typing your response, just tab to the next area.
  • Adobe Acrobat Reader, the most common program installed on computers to read PDF files does not allow you to work on the forms and save them with your information.  If you save them through Adobe Acrobat Reader and then open them up, they will be blank. If you would like to be able to save your forms with your information, you will need to download and install an alternate reader, such as the free Foxit Reader.
  • Foxit Reader (free version) allows you to save your work so that you do not have to complete the forms at one time.  Also, you would be able to save a completed copy for your records.

HOPE Applications
Students must complete the Free Application for Federal Student Aid (FAFSA) OR the GSFAPPS application to apply for any HOPE program.  Please view our HOPE Frequently Asked Questions. 

If you decide to apply for HOPE only, please review the GSFAPPS Instructions (in PDF format) which will make the application process easier for you. 

If you complete the GSFAPPS application at GAcollege411, you should check the Duck to verify that we have received your application.  Your GSFApps application may not automatically roll forward from one academic year to the next.   Please contact our office at FinancialAid@clayton.edu if you have any questions. 

If you complete the GSFAPPS application, you can only be evaluated for the HOPE Scholarship or the HOPE Grant. You must complete the FAFSA if you would like to also be considered for any type of federal aid (Pell Grant, Stafford Loans, Unsubsidized Loans, Federal Work-Study, etc.).
