Lost/Stolen Permit

Simply fill out the lost/stolen permit form below as completely as possible and click the submit button. Be sure to give all of your vehicle information. Additional permits are $10.00, and may be waived according to situation.

After your lost/stolen permit information has been reported. The information will be updated on the database system maintained by the Clayton State University Department of Public Safety.  Reminder, this will only update your vehicle information. To get a new permit, you will have to come in and complete a new registration form.  If you have any questions or concerns, contact the Clayton State University Department of Public Safety at 678-466-4050.
