2009 Alice Smith Staff Award Nomination Form
Nominations are due by Wednesday, March 11, 2009.

Eligibility:  Nominees must be a full-time regular staff employee with a minimum of one year of service from the time of nomination.

Full-time employees not eligible for the 2009 award are Sue Volker, Keya Reed-Jones, Roxanne Dilbeck and Sandra Starr.
